At Your Visit
When should you arrive for your appointment?
To keep your medical record current, there may be paperwork for you to complete prior to your appointment. Because of that, it is best to arrive at least 10 minutes before your scheduled appointment time.
Appointment Wait Time
We will do our best to keep our appointment schedule. However, please understand that not all patients require the same amount of time with the provider and that emergencies do occur; so some delays are unavoidable. We will do our best to keep you informed of delays. However, if you are waiting more than 30 minutes past your appointment time, please inform our registration staff. Your patience in these situations will be greatly appreciated.
Medications
Be sure to bring all medication bottles with you to each appointment. This will ensure that your medication list is accurate. Bringing the actual bottles is very important so that our nursing staff can confidently transcribe your prescription information in our electronic prescribing system. Insurance Cards
At each visit, please bring your insurance cards to present to our Registration Associates. Our team will be sure that we have the most up-to-date information in our billing system to ensure that your claims for services are sent to the right place. Changes in Your Personal Information
If you have changes to your name or address, please inform our registration staff at the time of check-in. They will be happy to update our system to avoid problems with insurance claims. This will also ensure that your medical record is corrected and billing statements are sent to the correct address.
Payment for Services
Co-pays and deductibles are due at the time of service. Additionally if there is a balance from prior visits, that balance will also be expected when you check-in with our Registration Associates. Payments can be made by cash, check, Visa, MasterCard, Discover or American Express.
