Billing & Insurance Information
Our charges for medical services are based on the nature of the procedure and the amount of time and professional skill involved. We request that charges be paid at the time of service. We will prepare and submit all primary and secondary claim forms on your behalf. In preparation to submit these claims, we will need current copies of all your insurance cards. Please provide a copy of your current insurance card at each office visit. You will be reimbursed for any credits to your account once insurance has paid. If payment is not made on the day of service, then charges for medical services are due and payable upon the receipt of your monthly statement. We accept cash, personal check, Visa, Mastercard and Discover.
If you are unsure whether or not your physician is a participating provider with your insurance or would like more information regarding what services are covered through your insurance, please contact the customer service number on the back of your insurance card for the most accurate, up-to-date information regarding your plan details.

Past-due accounts, on which no payment has been received (or contact made), will be referred to our collection agency. If unusual circumstances make it impossible for you to make payment according to the above schedule, please contact our business office to discuss the matter. This will allow us to work with you to keep your account in good standing.
